- Referral letter from your GP or other doctor (this may have been sent to Dr Nathan already).
- Medicare card, DVA card, Pension card.
- Private Insurance information and card.
- Copies of results of any tests relevant to the appointment:
- X-rays.
- CT scans.
- MRI scans.
- Other radiology.
- Blood tests.
- Urine test.
- A list of your medications.
- A list of your other medical problems and previous operations.
